    Stroke Recovery Care at Home

Coming home after a stroke is the point where families often feel most on their own. Reedsfield Care provides practical support at home during recovery: personal care, help moving safely around the house, meals, medication and the daily routine that rehabilitation depends on. Therapy is led by the NHS stroke and community teams, and our carers support what those teams have set out.

The first weeks after hospital discharge

Discharge often happens quickly. We can put support in place at short notice so someone is not going home to an empty house with a bag of medication and a leaflet. Early visits usually focus on getting up, washing and dressing safely, meals, fluids and medication.

We write a plan with you and review it often in the first month, because needs after a stroke usually change week to week.

    Supporting the therapy programme

Physiotherapists and occupational therapists set the exercises and the techniques. Our carers help people carry them out between sessions: practising a transfer the way the therapist showed, using the affected side rather than doing everything for the person, and keeping to the routine on the days when motivation is low.

Doing too much for someone after a stroke is a genuine risk. Carers are trained to support rather than take over.

    • Following the therapy plan between visits
    • Safe transfers and support with walking aids
    • Encouraging use of the affected side in daily tasks
    • Keeping to routine on difficult days

    Communication, swallowing and mealtimes

Where speech has been affected, carers give people time, use short sentences and simple choices, and follow the guidance from the speech and language therapist. The same applies to swallowing: modified textures and thickened drinks are prepared exactly as specified, and anything that worries us is reported to the team the same day.

    Confidence, mood and the family

Low mood and frustration after a stroke are extremely common and often the hardest part for the family. A consistent carer, seen at the same time each day, gives the week a shape and gives the person someone to talk to who is not also worried sick about them.

We keep families updated honestly, including when things are not going well.

    Frequently asked questions

How quickly can care start after discharge?

Often quickly, including evenings and weekends. Call 01784 740078 as soon as a discharge date is being discussed and we will tell you what is realistic.

    Do you provide therapy?

No. Physiotherapy, occupational therapy and speech and language therapy are led by NHS or private clinicians. Our carers support the plan those clinicians set.

Can care be reduced as recovery progresses?

Yes, and it often is. You can step hours down or up with a week's notice.

Can you help with two people, if a spouse also needs support?

Yes. Care can cover a couple, and with live-in care one carer often supports both partners.

    Is overnight support available?

Yes, including sleep in nights, waking nights and full live-in care.
