    End of Life Care at Home

    Most people, asked, say they would rather die at home. Reedsfield Care provides personal and practical care at home in the final months, weeks and days, working alongside the hospice team, district nurses and GP who lead on symptom control. Our carers do not provide medical treatment. What they provide is presence, comfort and the ordinary care that lets someone stay where they want to be.

    What care at the end of life involves

    Comfort comes first: careful personal care, mouth care, repositioning, clean bedding, quiet company. Carers keep a clear record for the district nurses and call them promptly when pain or symptoms change, so that clinical support arrives quickly.

    As things progress, care usually increases: more visits, then overnight support, then in many cases a live-in carer or waking nights so the family are not awake every night themselves.

    Working with the hospice and district nurses

    Pain relief, anticipatory medicines and symptom management are led by the district nursing and hospice teams. Our carers work to the same plan, notice changes between visits and escalate them, and make sure that when a nurse arrives the practical side is already in order.

    Supporting the family

    Families do not need to be told to be strong, they need sleep and a hand with the practical things. Carers take on the washing, the food nobody is eating, the changing of bedding at three in the morning, and give relatives permission to sit down and just be a son, a daughter or a husband again.

    We keep families informed honestly, and we do not pretend things are better than they are.

    Can you provide pain relief?

    No. Pain relief and anticipatory medicines are managed by district nurses, the hospice team and the GP. Our carers support comfort, personal care and daily living, and escalate changes promptly.

    Can someone be cared for at home right to the end?

    Very often, yes, with the right combination of hospice and district nursing input and care in the home. We will be honest with you about what is realistic in your situation.

    Is overnight care available?

    Yes, including sleep in nights, waking nights and full live-in care.

    Do you support the family afterwards?

    Carers and care managers stay in touch in the days after a death and help with the practical steps. Bereavement counselling is provided by the hospice or GP.
